Patient Experience Survey

Each year, our PCN administers a Patient Experience Survey to collect feedback from our patients.  

Introducing the survey

Our first Patient Experience Survey launched in 2017, with the goal of connecting directly with our patients who had appointments with our clinical team members (nurses, psychologists, etc.). Since then, the survey has helped us identify areas of strength and opportunities for improvement.  

As a PCN, our goal is to provide the best support possible for doctors in our membership and their patients. This evaluation process is an important part of ensuring our programs and services are meeting the needs of our community.  

In 2024, the survey gathered data from patients of our Registered Psychologists, Primary Care and Senior Services Registered Nurses (RNs), and Social Workers. We received a total of 477 responses to the survey.

Program highlights

Mental Health Program highlights

Our Mental Health Program includes Registered Psychologists and Social Workers who provide support through psychotherapy appointments, community resource connections, and more.  

The 2024 Patient Experience Survey showed:  

  • 92 per cent of respondents with an evening or weekend appointment with a Registered Psychologist or Social Worker agreed or strongly agreed that these options allowed them to access services they otherwise would not be able to. 
  • 87 per cent of respondents who had an appointment with a Registered Psychologist reported feeling better prepared to deal with daily challenges because of their visit(s). 
  • 83 per cent of respondents who had an appointment with a Social Worker said their provider connected them to or helped them apply for required services. 

Nursing program highlights

Our Nursing Program includes Primary Care RNs and Senior Services RNs who provide support to help patients manage chronic diseases, navigate life changes, and much more.  

The 2024 Patient Experience Survey showed: 

  • More than 95 per cent of respondents who had an appointment with an RN said their provider explained things in a way that was easy to understand, involved them in their care decisions, and listened carefully to their needs. 
  • About 95 per cent of respondents who saw an RN said that they have the skills and knowledge to manage their health. 
  • 94 per cent of respondents who had an appointment with a Senior Services Registered Nurse were very satisfied or satisfied with the care they received. 
  • 88 per cent of respondents who had an appointment with a Primary Care Registered Nurse were very satisfied or satisfied with the care they received in the past six months. 

Overall findings

We are pleased to see that our programs and services continue to connect our community with the resources and support they need to promote their well-being. 

2025 results will be expanded to include feedback about our Dietitian Program, Physiotherapy Program, Senior Assessment Clinic, and Primary Care Centre.
