Annual Report 2024-25
From our Leadership
Each year, our annual report provides a snapshot in time: A year’s worth of achievements and efforts to move our Primary Care Network (PCN) forward.
It also serves as a reminder of all the work that brought us to this point. Each improvement, project, and result building on our past to better serve our patients, the doctors in our membership, and our PCN.
Always listening and looking to improve, we used stakeholder engagement to offer new patient care programs and enhancements, embraced new learning opportunities and technology, continued to provide an array of helpful programs and services, and more.
Our 2024-25 Annual Report showcases many examples of our PCN’s work across our four priority areas in the last fiscal year. Please use this website to learn about how we contributed to primary care and improved as a PCN.
Primary care is the frontline of our healthcare system, and we thank everyone who is a part of it. Our members, patients, and teams all play vital roles in creating a healthier future.
